5 Grant & Per Diem position is within the Behavioral Health Care Line (BHCL) located at the Albuquerque, New Mexico Veterans Affairs Health Care System (NMVAHCS) and will report to the Durango Contracted Community Based Outpatient Clinic in Durango, Colorado.
The incumbent in this position functions as the Medical Center's Liaison/Case Manager for Homeless Veterans residing in the Durango, Colorado area and are part of Albuquerque VAMC's Healthcare for Homeless Veterans Program, Department of Housing and Urban Development/VA Supportive Housing (HUD-VASH).
The incumbent is responsible for monitoring all phases of programming and coordination of case management services for veterans receiving housing provided through HUD Section 8 housing vouchers.
The incumbent is also the GPD Liaison which functions as the Medical Center's Liaison/Case Manager for VA's, Homeless Providers Grant and Per Diem Program.
The incumbent is responsible for monitoring and coordination of services provided by community-based programs funded under VA's Homeless Providers Grant and Per Diem Program.
The ideal applicant will be an independently licensed social worker with a variety of experiences.
, The Social Worker applicant must have the knowledge, skills and ability to complete psychosocial assessments, planning and evidenced based treatment; psychosocial case management; discharge planning coordination; health care coordination; suicide risk assessment and follow-up, individual, family, and group counseling; patient and family education.
